Final Warning: Students in danger of being removed from courses, housing
This message was sent to students who have not provided the required documentation for COVID-19 vaccination.
Dear Wake Forest student,
This message is to notify you that Wake Forest has not yet received required COVID-19 vaccination documentation from you, and you are not compliant with the University’s COVID-19 Vaccine Policy.
If you have submitted this documentation in the last 24 hours, thank you for doing so. Your submission is being reviewed.
If you submitted documentation of your vaccination status prior to the last 24 hours, please check your Immunization History and Secure Messages in the Student Health Portal immediately. If a COVID-19 vaccine does not appear in your Immunization History, you must resubmit your documentation by August 1.

If you have not submitted documentation and you intend to remain enrolled in campus programs in the fall of 2021, you must:
- submit your documentation through the Student Health Portal immediately;
- OR request a medical or religious exemption;
- OR, if you are an international student, email traceshs@wfu.edu and state that you are unable to access an FDA-authorized vaccine before Aug. 1 but intend to comply with the requirement.
If you do not do any of the above actions, you will be removed from your courses and from your University-assigned housing (if applicable).
NOTE: If you have received or will receive only the first dose of the Pfizer or Moderna vaccine by August 1, please submit documentation of the first dose. Submitting documentation of the first dose by August 1 will allow you to remain enrolled in classes while you are waiting to receive the second dose of the vaccine within the time frame required by the Student Health Service and until you are considered fully vaccinated.
