Dear Colleagues,

Last week, Wake Forest University Human Resources provided faculty and staff with a COVID-19 vaccination distribution update. While vaccine shortages have been widely reported and the university cannot predict when supplies will increase, we are proactively preparing for the possibility of increased production and distribution of doses. In the event that the university has the opportunity to secure vaccination appointments for colleagues in the future, Human Resources is working to identify those who would be eligible for Phase 1 Group 3 and most at risk for exposure.

To help us in this process, all active full- and part-time faculty and staff should accurately self-identify their personal work circumstances by completing the COVID-19 Vaccination Planning – Employee Survey. If you have already received a COVID-19 vaccine, we ask that you still complete the survey. Inactive retirees and dependents should not complete this survey. Student workers will receive a separate communication.

You will receive an email to complete this survey later today from Human Resources (no-reply@qualtrics.wfu.edu) with the subject line COVID-19 Vaccination Planning – Employee Survey. Please complete the survey as soon as possible and no later than Friday, February 12. This survey will be used for planning and allocating resources for upcoming vaccination clinics offered or coordinated by the university. Your responses will be recorded and Human Resources will communicate with you as you become eligible for an upcoming vaccination opportunity, in accordance with NCDHHS eligibility guidelines. If you are unsure of what vaccine group you are eligible for, we suggest using the NCDHHS Find My Vaccine Group tool.

Continue to closely monitor your email and respond quickly to any notifications of vaccination opportunities. We also encourage you to take advantage of any other opportunity you may have to get vaccinated. While the vaccine is not required by Wake Forest University, it is highly recommended that all employees get vaccinated.

If you have questions related to COVID-19 vaccine distribution and planning, please review these Frequently Asked Questions or submit your question(s) to Human Resources using this form.

We appreciate your patience as we navigate this process. We look forward to promoting the health and wellbeing of our Wake Forest community.

Sincerely,
Wake Forest University Human Resources
