The University sent several different messages to students related to non-compliance with the pre-arrival requirements. The message information varied depending on the issue. Below are two samples, one for on-campus students and one for off-campus students.

On Campus

Dear student,

Your Deacon One Card has been deactivated. You are receiving this message because you have not:

  • Submitted documentation of a flu vaccination or received an exemption
  • Submitted documentation of a negative COVID-19 test or a positive test dated on or after Oct. 24, 2020

These requirements were originally outlined in an email sent to you on Dec. 17, 2020, and included in several subsequent reminders.

More information on the flu requirement — including how to submit documentation or request an exemption — is available on the Student Health Service website.

  1. Before you arrive, you can get a flu vaccination and submit results or bring them with you to campus.
  2. If you have not received a vaccination or notice from Wake Forest that your documentation or exception has been accepted before arriving on campus, you will need to complete the steps in the “Upon Arrival” section below.

Next steps for testing

  • Submit Documentation
    • If you have documentation, please submit it immediately.
    • If you are waiting on documentation, submit it as soon as you receive it. Please understand that depending on when Wake Forest receives your documentation, your card may still be deactivated when you arrive. You should bring a copy of your negative results and go through the “Upon Arrival” process outlined below.
  • Get Tested Immediately
    • If you choose, you can get tested before arrival to campus. Details on acceptable tests and possible sources are available here.
    • If you receive results, submit them and bring them with you. If you test positive, submit your results but do not come to campus. Isolate yourself and do not return to campus until you have met CDC criteria for release from isolation.
    • Please understand that if you do not receive results before arriving on campus, you will need to go to quarantine upon arrival as described below. However, being tested now could shorten your time in quarantine.
  • Get Tested on Campus
    • If you are awaiting results or have not received a test, you can be tested by Wake Forest as part of your arrival process (see below). However, you will not be able to access any other part of campus, including your room to store belongings, and will be sent to quarantine (see below) to await results. Please note this testing will be billed to either your student insurance or your student account at the cost of $120.

Upon Arrival

  1. If you have not received notice from Wake Forest that you are cleared before arriving on campus, you must report to the Reynolda Road Gatehouse (No. 51 on the campus map) between noon-4:30 p.m. on Jan. 23 (for students moving into their residence hall for the first time this academic year) or Jan. 24-25 (for students who lived on campus this fall). If you plan to arrive on Jan. 26, you will receive instructions by email on Jan. 25 on how to access campus.
  2. You will be directed to the parking area at the Byrum Welcome Center (No. 53 on the campus map) to address this issue with University staff. Testing and vaccinations will be available. Please note that these will be billed to either your student insurance or your student account at the cost of $30 (flu) and $120 (COVID test).

Quarantine

  • Students who do not have documentation will need to quarantine after arrival until testing negative or, if they test positive, until they meet CDC criteria for release from isolation.
  • Quarantine will be at the Best Western Plus or Hawthorne Inn.
  • You will not be able to access your room to move in any belongings before quarantining.
  • If you have not taken a COVID-19 test before arrival and are tested when you arrive on campus, quarantine will last at least 24 hours and perhaps five days or more, depending on how soon test results are being processed.

Off Campus

Dear student,

Your Deacon One Card has been deactivated. You are receiving this message because you have not:

  • Submitted documentation of a flu vaccination or received an exemption
  • Submitted documentation of a negative COVID-19 test or a positive test dated on or after Oct. 24, 2020

This documentation was due on Dec. 15, 2020 and is required by University policy. More information — including how to submit documentation or request an exemption — is available on the Student Health Service website.

Next Steps for Flu Vaccination

  1. You need to receive a flu vaccination and submit documentation through the SHS portal. You can arrange for your own flu vaccination or receive one from Wake Forest from Tuesday, Jan. 26, through Friday, Jan. 29, between 8:30 a.m.-4:30 p.m. at the former Museum of Anthropology (No. 29 on the campus map). You should enter campus through the University Parkway Gatehouse (No. 50 on the campus map) and park in Lot X. Please note that this will be billed to either your student insurance or your student account at the cost of $30.
  2. Once your documentation is accepted, your Deacon One Card will be reactivated.

  • Get Tested after Arrival
    • You can be tested locally after arriving. You then will need to submit documentation of a negative test. If you test positive, submit your results but do not come to campus. Isolate yourself and do not return to campus until you have met CDC criteria for release from isolation.
  • Get Tested by Wake Forest
    • On Friday afternoon (Jan. 22), you will receive an email with information about how to sign up for testing through the University. Appointments will be available for Jan. 26-29.

You must be cleared by Student Health for both requirements to have your Deacon One Card reactivated.
